What this notice covers
This notice describes the Kids Life Inc. website and our general inquiry form. Program applications, clinical services, and client records may have separate privacy information provided during intake.
Information you choose to send
Our general inquiry form asks for your name, the topic of your inquiry, an email address or phone number, your preferred contact method, whether it is safe to leave a message, an optional brief message, and permission to contact you. If you email or call us, we receive the information you provide that way.
Please do not send diagnoses, treatment history, Medicaid IDs, details of violence or trauma, or other sensitive information through the general inquiry form or ordinary email. A parent or guardian should contact us about a child; please do not include a child's sensitive details in a general inquiry.
How we use and handle inquiries
We use general inquiry details to respond, route your question to the appropriate Kids Life Inc. program, and follow up on the service or partnership you asked about. People helping Kids Life Inc. respond to inquiries may access the information needed for that work. The form is hosted by Google Forms, which processes information submitted through it. We may also disclose information when required by law.
Your choices and questions
You can contact us by phone instead of using the form. To ask about a submitted inquiry, request a correction, or request deletion where possible, email info@kidslifeinc.org or call 720-689-4469. If the inquiry needs clinical intake or urgent assistance, please use the appropriate program intake or emergency service rather than this general form.
